To better understand the benefits and risks of different DOAC dosing schedules, we should address the current research gaps with Longer-term head-to-head studies with real-world data of patient adherence and detailed subgroup analyses of both doses. Current evidence combines different DOACs and dosing regimens together, but to better understand and clarify differences in efficacy, safety, and adherence of each regimen, we need direct, long-term randomized trials comparing once daily and twice-daily doses. Additionally, for tailoring dosing to individual patient profiles, we need to stratify outcomes by factors such as age, renal function, comorbidities, and prior history of bleeding or stroke, as these factors influence both dosing decisions and risk assessment. One other important aspect is to understand the Drug-Drug Interactions of DOACs with other drugs. Elderly and multimorbid patients often take multiple medications, increasing the risk of drug-drug interactions that may affect DOAC efficacy and safety. By addressing these gaps, we can understand the real benefits and risks. of each regimen and tailor the appropriate dosing for each patient subgroup.
