For patients with recalcitrant plaque psoriasis, combining narrowband UVB phototherapy with biologic agents produced outcomes far beyond what either method delivered alone. A patient who had cycled through multiple topical and systemic medications with limited relief saw rapid clearance when phototherapy was introduced alongside a biologic. Within weeks, lesions that had resisted years of conventional management diminished in size and inflammation, and the patient reported reduced itching and improved sleep. Compared to prior treatments, the dual approach not only accelerated visible improvement but also sustained remission longer between flares. The patient's quality of life improved significantly, with greater confidence in daily activities and fewer absences from work. The experience reinforced the value of integrating established modalities with newer targeted therapies, showing that innovation often lies in thoughtful combinations rather than in replacing one treatment with another.
