A team involving pulmonologists, nurses, respiratory therapists, and dietitians worked together to enhance the patient's care plan. The respiratory therapist introduced breathing exercises to improve lung function, while the dietitian provided nutritional advice to help maintain the patient's weight and energy levels. Nurses ensured the patient adhered to medication schedules and educated them on recognizing early signs of exacerbation. The results were significant. Studies indicate that Interprofessional collaboration can lead to a 30% reduction in hospital readmissions for COPD patients (McHugh et al., 2016). In this case, after implementing the collaborative care plan, the patient experienced a 40% decrease in exacerbations over six months, leading to an improved quality of life. This example illustrates how teamwork not only enhances patient care but also optimizes outcomes through shared expertise and comprehensive support.
