I believe that goal-setting in therapy is a collaborative process that is helpful to discuss during the first session. Sometimes clients can be overwhelmed about goal setting, so I approach it by expressing that our treatment plan is a "living document". I reaffirm to my clients that the treatment plan can change and evolve over time, just as humans do. Furthermore, I utilize the discussion of goal-setting to focus on the present moment with mindfulness. It can be difficult for some people to envision long-term goals. I have found it beneficial to approach the question around goal setting as, "What are your goals for therapy that are coming up at this moment?". Both of these approaches help to relieve the pressures around goal-setting, while also teaching therapeutic concepts in the process.
When guiding clients through the goal setting process, my approach emphasizes clarity, measurability, and motivation. In approaching the process of goal setting, it's essential to begin by exploring one’s values and aspirations across various life domains. This may involve identifying goals within areas such as family, relationships, career, health, and personal interests. A critical step in this process is constructing an activity hierarchy to rank goals from easiest to most challenging, ensuring a clear understanding of the progression towards each goal. Once goals are identified, the next phase involves fine-tuning them to enhance the likelihood of success. This includes making goals observable, measurable, and concrete goals, with clearly defined endpoints. Concrete goals provide clarity and allow for easy measurement, while endpoints mark achievement and provide opportunities for celebrating progress. Additionally, I encourage individuals to frame their goals in an approach-oriented manner, focusing on what one aims to achieve rather than what to avoid. Approach-oriented goals tend to enhance motivation and propel individuals forward towards tangible achievements. Furthermore, breaking down larger goals into manageable sub-goals is another crucial step of goal setting. This strategy prevents individuals from feeling overwhelmed and boosts their confidence by providing smaller victories along the way. Moreover, visualizing success at each step of goal pursuit can enhance motivation and perseverance. By following these steps, individuals can set meaningful, achievable goals aligned with their values, increasing the likelihood of successful goal attainment and personal growth.
Discussing a patient's goals are an invitation for them to think about their ideal self. It's important for them to evaluate their situation and to find balance in not just working on them. I encourage them to build a roadmap and examine what resources with tools that they need to accomplish them. Exploring what strengths they already have and use them. When patients create actionable, realistic steps they can also understand any obstacles that they may see. Holding oneself accountable with goal setting, addressing any fears, identifying any habits you need, clearing your mind of self doubt, staying on track and ultimately celebrating your progress are all important things to remember in this process. Goals need to be realistic. Some goals are daily, weekly, monthly and some take years.

However, when we are empowered to create our own goals, we are much more likely to make genuine attempts to achieve them.
