As a case manager, handoff is an integral component in care coordination. I always want my work to reflect thorough details, so when another team member needs to take over transition is smooth and cohesive. My notes tell a story of the patient's medical journey. With a thorough comprehensive handoff, continuity of care is easy. In our electronic health record (EHR) there is a space that allows for all the essential information needed for care to continue without extensive review. One key element that distinguishes the hospital case manager is handoff also continues into the community. I provide the most current status of the patient to their insurance case manager, as well as any medical services already established such as nursing and durable medical equipment (DME) and primary care physician (PCP) all receive discharge paperwork, new orders, changes and ongoing tasks that need follow-up. Well structured and timely care coordination and handoff is the cornerstone of my practice. I believe that when there is engagement within the patient's clinical network helps streamline care, flow process, diminish readmission and ultimately facilitates better outcomes overall. When this is done on a continuous basis, incorporated within the case managers nursing process-it becomes an unconscious practice that every patient benefits from.
To improve the patient handoff process, we've started using digital "sticky notes" within our electronic medical record (EMR) system. These sticky notes highlight crucial patient information such as allergies, medication changes, and special care instructions, making it easily visible to all team members. This quick-reference method ensures that vital details aren't missed during handoffs. Since implementing this, we've noticed a significant decrease in communication errors and a smoother transition of care. The sticky notes have made our handoff process faster and more reliable, ultimately enhancing patient safety and satisfaction.
