What's one method you've used to ensure accurate record-keeping in a hectic veterinary environment?

For me, writing down important findings on my physical exam as I do it is a must. This alleviates the stress of having to remember each important finding and knocks out records that I potentially have to write later. I usually state to my veterinary technician what I want written on my Objective portion of my SOAP while performing my physical exam. I find this often knocks out two birds with one stone in the midst of a hectic workday.
